Application Process for Agreement for Acceptance of Specimens Collected by Independent Phlebotomists for Laboratory Testing by Pathology and Laboratory Medicine

The steps below outline the process for establishing an Agreement for Acceptance of Specimens Collected by Independent Phlebotomists for Laboratory Testing by Pathology and Laboratory Medicine.

Step 1: Review the Agreement for Acceptance of Specimens Collected by Independent Phlebotomists for Laboratory Testing by Pathology and Laboratory Medicine to ensure an understanding of the requirements within the agreement.

Step 2. Open the Independent Phlebotomy Application Form and save on your desktop. Complete form . Please ensure all requested information is provided to avoid delays in processing. Forward completed form as an email attachment toIPOffice@nshealth.ca . Keep a copy for your records. 

Step 3. The requestor will be notified if the application is not approved or incomplete.

Step 4. The application will be processed and a contract will be drawn up and emailed to the requestor. Print and review to ensure the information is complete and correct. This is now the “Original Agreement.” Ensure signatures and compliance to the terms of the agreement (e.g TDG training, access to standards, etc).

Step 5. Signed contracts and supporting documentation must be submitted to Pathology and Laboratory Medicine. Options to submit are as follows: Scan and email to IPOffice@nshealth.ca Fax: 902-473-7038 Mail, courier or hand deliver to Department of Pathology and Laboratory Medicine Atn : IP Office Rm. 126 Mackenzie Bldg 5788 University Avenue Halifax, NS B3H 1V8

Step 6. NSHA and IWK authorized signature will be obtained giving final approval. . The Agreement will be activated and site management will be notified with the activation date. A copy of the signed agreement will be forwarded to the requestor by email, along with notification when collection can begin.

The process to review, approve and activate a new agreement can take four to six weeks. Please ensure all requested information is provided to avoid additional delay.
