The community outreach assessment support and treatment team provides services for persons with both intellectual disability and also mental illness. These services include:

  • Assessments including a detailed report and recommendations
  • Short-term treatment and support with a view to discharge client back to his/her family doctor
  • A plan for leaving the hospital and follow up after patients leave the hospital 
  • Working in collaboration with family physicians and other community health care providers to facilitate the recovery of individual clients.

How do I get an appointment?

A referral from your doctor or psychiatrist is required.

Please note: The COAST team has a specific referral package. Please download the referral package online. After the submission of both forms, the COAST team will review your completed referral package and contact you with a decision.
