The Integrated Chronic Care Service (formerly known as the Nova Scotia Environmental Health Centre) provides comprehensive assessments and care planning for individuals with complex chronic conditions such as:
  • Chronic fatigue syndrome (CFS)
  • Central Sensitivity Syndrome (CSS)
  • Environmental illnesses (chemical intolerance, chemically triggered conditions, allergic conditions, building related illness)
  • Fibromyalgia (FM)
  • Functional neurologic and gastrointestinal syndromes
  • Multiple chemical sensitivity (MCS)
  • Multiple chronic conditions (multi-morbidities)
  • Treatment resistant pain conditions
Our model is based on the premise that chronic disease outcomes are better managed by addressing the needs of the whole person with a focus on self-management support.  With a team of interdisciplinary health care professionals, we integrate the physical, psychosocial and environmental needs of individuals by focusing on the person instead of the disease(s)/condition(s), improving functional health and quality of life.  Our care team includes: physicians, occupational therapists, a nurse practitioner, registered nurse, licensed practical nurse, a psychologist, psychotherapist and a dietitian.

DIRECTIONS
Appointment required.  Regular hours are Monday to Friday 8:30 am to 4:30 pm.  Some evening sessions provided.  Closed on statutory holidays.

How do I get an appointment?

The Integrated Chronic Care Service receives referrals from within Nova Scotia, across Canada and internationally. We accept referrals from family physicians. We also accept referrals from nurse practitioners within Nova Scotia.
