Independent Phlebotomy Application
Thank you for your interest in providing independent phlebotomy services with NSHA and the IWK. Pathology and Laboratory Medicine is in the process of reviewing how we work with independent phlebotomists to ensure the highest possible level of quality and patient safety. As a result, we will not be expanding services or entering into any new agreements with independent phlebotomists at this time.
However, we are processing applications for independent phlebotomists who currently hold agreements that expired April 1, 2016 and have been extended until the applications are completed. We are also establishing contracts with independent phlebotomists currently collecting in Western, Eastern and Northern Zones. See the Nov. 8, 2016 memo “Requirement to enter into a contractual agreement with NSHA and IWK” or attend an information session being hosted by Pathology and Laboratory Medicine.
To help understand the obligations associated with meeting the terms of the contract, please review the Agreement for Acceptance of Specimens Collected by Independent Phlebotomists for Laboratory Testing by Pathology and Laboratory Medicine.
To learn more about the application process see Application Process for “Agreement for Acceptance of Specimens Collected by Independent Phlebotomists for Laboratory Testing by Pathology and Laboratory Medicine.“
