If you are seeking a receipt of your COVID-19 vaccination, visit https://novascotia.flow.canimmunize.ca/en/portal for instructions.

For all other vaccine records

VaxRecordNS* is now available to obtain select vaccination records using the digital solution.  Please go to https://vaxrecordns.nshealth.ca and follow the instructions to set up an account and access VaxRecordNS*.

Or, you may also contact your local Public Health office for all vaccines.  Please use this form and follow the instructions to submit your request.

If you are a parent requesting information for your child, if your child has been determined to be a mature minor, then your child must sign the authorization form requesting their immunization records.
