Care Coordination Centre leader shares success story with national audience
When you're not feeling well, timely access to the right care can make all the difference.
That’s why Nova Scotia Health introduced the Care Coordination Centre (C3)—a province-wide system designed to improve how patients flow through the healthcare system.
At a recent panel discussion, titled Command Centres as Enabling Technology for Provincial Access and Flow — Andrea Muenster, senior director of C3, shared how our innovative system is transforming the way we help Nova Scotians along their healthcare journey.
Hosted by the Health Information and Management Systems Society (HIMSS), a nonprofit organization committed to advancing health through technology and collaboration, the panel discussed how command centres like C3 are leveraging digital data dashboards with real-time information on patients and health system resources to connect patients with the care they need sooner. The conversation highlighted how these efforts align with Nova Scotia Health’s strategic priorities, including digital transformation, cultural change, operational excellence, and the long-term vision for the future of healthcare.
“Sharing Nova Scotia Health’s work with C3 through platforms like HIMSS allows us to highlight how we’re leveraging data to prioritize patients and deliver truly patient-centred care,” said Muenster. “It also opens the door to learning from other health systems, helping us continuously evolve and stay aligned with emerging healthcare trends.”
Muenster shared with the panel that C3’s technology empowers healthcare teams to make faster, more informed decisions while also reducing the manual workload on frontline staff. This leads to shorter wait times, smoother transitions between care settings and a significantly improved experience for patients and their families. It’s a powerful example of how innovation is advancing the way healthcare is delivered every day.

C3 uses real-time data and advanced technology to give healthcare teams a clear picture of what’s happening across the province, from available hospital beds to patient transfers and appointment scheduling. Whether it’s helping book final appointments before discharge, co-ordinating transfers between hospitals, or making sure beds are available when and where they’re needed, C3 is about making the system work better for patients and the healthcare teams caring for them.
