The Seniors Community Health Team is a team of health care providers dedicated to the health, safety, and well-being of older adults. They visit older adults in their homes in Lunenburg and Queens Counties. The goals of the program are to help to improve older adults' quality of life, and help them to stay healthy and independent, at home and in their community.
