The Seniors Community Health Team is a team of health care providers dedicated to the health, safety, and wellbeing of seniors. They visit seniors in their homes in Lunenburg and Queens Counties.
The Seniors LINCS Program visits seniors in their homes in Annapolis and Kings Counties and helps seniors stay in their homes and communities independently for as long as possible.
