Transitional health helps bridge the gap between hospital and home, with a focus on active recovery through the Home First philosophy and practice.

West Bedford Transitional Health supports patients aged 18 and older who no longer require acute care but need assistance in transitioning back to the place they call home or to other forms of care in the community. Patients will be referred to West Bedford Transitional Health by Nova Scotia Health based on clinical criteria.

Parking

There is free parking at West Bedford Transitional Health.

Wi-Fi

While you are at Transitional Health, you will have free access to wireless Internet services (Wi-Fi).
You are welcome to bring small electronic devices (like a laptop, a tablet, or a cellphone).

If you would like to listen to music or audiobooks, bring headphones so you do not disturb others.

What happens at West Bedford Transitional Health?

At West Bedford Transitional Health, Shannex® care teams work with patients to create a restorative action plan that prepares them to return to the place they call home safely and with confidence.

Upon admission, each patient’s health, care and social support needs are assessed, and patients identify their personal goals for their stay. Together, these elements inform a patient’s estimated date of discharge (EDD) and define the objectives that need to be achieved before a patient is discharged.

Throughout their stay, patients are active participants in their recovery. Movement and social interaction are encouraged, and progress toward goals is recognized and celebrated.

Transition to Home Apartments provide a home-like environment where patients can test their independence and build confidence with daily activities. Prior to discharge, patients work closely with the Nova Scotia Health Community Outreach team to ensure the appropriate supports and services are in place for success at the place they call home.

Upon discharge, patients may receive follow up services from this team based on assessed needs.

Loved ones and support persons

Your loved ones and support persons are an important part of your health care team. We encourage their presence to help you plan for your discharge.

Is there a cost for Transitional Care?

There is no cost while staying at West Bedford Transitional Health. In some cases, however, your medical or personal circumstances may change and going home is no longer possible. If it is determined you require direct placement to a long-term care facility, you will be charged an accommodation fee similar to a long-term care facility while you wait

Personal belongings

  • Please bring clothes, footwear, and personal care items to make sure you will be able to participate in your daily care plan.
  • Do not use scented products.
  • Nova Scotia Health is scent-free.
  • Do not bring extra furniture or large personal
    items (like chairs, storage bins).
