Community Health Teams

The Community Health Team is a service of Nova Scotia Health in partnership with the IWK.  The Community Health Team offers FREE in person group wellness programs and navigation services in the greater Halifax area to those age 18 and older.  Some of these programs are also offered virtually through Zoom for Healthcare.  Program topics include:

  • Healthy Eating         
  • Mental Wellness
  • Physical Activity
  • Parenting
  • Managing Risk Factors

FREE Wellness Navigation (greater Halifax area)

Wellness navigation can help you identify what’s getting in the way of your health, and connects you to available resources in your community and online.  It is a free non-urgent service for individuals, families and youth offered by health professionals.  Navigation is available to residents of the greater Halifax area with a valid NS health card.      

Appointments can take place by phone, Zoom for Healthcare, or in person.  For more information, call 902-460-4560 ( option 1 for Bedford, 2 for Chebucto, 3 for Dartmouth or 4 for Halifax).
