You have the right to access and request a copy of your personal health information from Nova Scotia Health Authority.
There are limited exceptions to your right of access. For example, you may not be given access to information that was collected during an investigation or that includes personal health information about another individual.
There may be fees associated with this service based on the region you submit the request to.

How do I request the release of my health records from NSH?
You will need to know which of the former Nova Scotia District Health Authorities currently holds your information.
One you have determined that, fill out this request form below and submit it to the relevant contact person.

Below is a list of the most common associated fees.
Request Fees (HST included):
- Visit History
$11.50 - Processing Fee
$34.50 (includes processing request, up to 30 minutes of review/prep time, copying up to 25 pages)
Additional Fees that may be incurred based on the type and size of your request (HST WILL BE ADDED TO ALL LISTED FEES):
- Preparation of Record
$10.00 - Viewing of a Record
$ 6.00 for each additional 30 minutes after the first 30 minutes - Diagnostic Images on a CD
$ 10.00
Requests for access to records are also subject to additional fees if requestor is seeking expedited access to off−site records, delivery by courier, or out of country mailing.

Contact the Public Health office in your area to learn how to access your immunization records.
If you are a parent requesting information for your child, if your child has been determined to be a mature minor, then your child must sign the authorization form requesting their immunization records.
