How do I prepare for my appointment?

Organize Your Documents: Ensure you have your health card and a list of all medications and supplements you are currently taking. Keep these items easily accessible to provide to your healthcare team.

Compile Your Questions:
Take some time to think about any questions or concerns you may have regarding your health, weight, or upcoming procedures. Writing them down beforehand can help ensure you address everything during your appointment.

Complete Patient Intake Form:
Fill out the patient intake form thoroughly before your visit. This form provides valuable information about your health and diet history, enabling your healthcare team to better understand your needs. Remember to send the completed form to the team ahead of time and bring a copy with you to the appointment.

Prepare for Physical Assessment:
Anticipate that your healthcare team will conduct a physical assessment, which may include measurements such as height, weight, and blood pressure. Dress comfortably and be ready to participate in the assessment.

Review Health History:
Reflect on your health history, including past surgeries and any changes in your medications or supplements. Being informed about your own medical background will help facilitate discussions with your healthcare providers.

Clarify Your Goals and Concerns:
Think about your personal health goals and any concerns you have related to your weight and overall well-being. Communicate these goals and concerns clearly to your healthcare team during your appointment.

Participate in Treatment Planning:
Be actively engaged in the treatment planning process. Collaborate with your healthcare team to develop a personalized plan that aligns with your goals and addresses your individual needs. This may involve considering surgical options, as well as other supportive measures such as dietary changes, increased physical activity, and mental health support.

Embrace Education and Preparation:
Embrace the educational resources provided by your healthcare team to prepare for your surgery and recovery. Take advantage of information sessions and materials that outline what to expect before, during, and after the procedure. Follow any pre-operative instructions carefully to optimize your outcomes.

What will happen at my appointment?

Once you have been referred by a family doctor, nurse practitioner or a specialist, the Bariatric Clinic will send you an intake form.

You will then have a consultation with an obesity specialist and a registered dietitian.

If needed, you may also have a consultation with a psychiatrist, social worker, psychologist, specialist or be referred on to community teaching and for some additional health tests.

After that is completed, there will be a follow up from an obesity specialist, and a referral to bariatric surgery* if needed.

*Bariatric surgery, commonly known as weight-loss surgery, is a proven intervention for individuals struggling with severe obesity, particularly when diet and exercise alone have been ineffective.

While bariatric surgery can significantly improve health and quality of life, it is a major medical procedure that requires lifelong commitment to healthy eating, regular exercise, and ongoing medical follow-up for lasting success.
