Pathology and Laboratory Medicine – Requisitions and Forms

Requisitions

A lab requisition is a document your health care provider might give after your visit. It asks us to do specific lab work, like blood or urine tests. The goal is to get a better idea of what's going on and help pick the right treatment or next steps for you.

Most lab requisitions now use a digital system, instead of being a printed copy. If yours is printed, please bring it with you to your appointment.

Health care providers who need printed requisitions can get them at no cost. If you have questions or need to know more about how to get blank copies, please contact us.
