Cardiac Transplant & Advanced Heart Failure Clinic

The Cardiac Transplant Clinic/Advanced Heart Failure Clinic offers specific care for patients with advanced heart failure in the Atlantic provinces.    

This clinic is for pre/post cardiac transplant patients and those requiring mechanical circulatory support by a designated team of heart failure specialists.

The Advanced Heart Failure and Transplantation Service consist of inpatient and outpatient services. Inpatient services consist of a ward service with virtual beds, and a consulting service available to provide assessment and therapeutic recommendations on patients admitted to general cardiology or cardiac surgery.  The transplant cardiologist will decide who are appropriate patients for admission to the inpatient service.

New Health Information System

Nova Scotia Health's new One Person One Record system has rolled out at facilities in the Halifax Regional and West Hants municipalities, and some provincial Mental Health and Addictions Program services.
